A Clinical Analysis of the Extrahepatic Bile Duct Cancer

Abstract
The records of 39 patients with extrahepatic bile duct (EHBD) carcinoma who received operation at the Department of Surgery, yonsei University Wonju College of Medicine from Mar., 1982 to Feb., 1991 were reviewed. Twenty-seven of 39 patients were

with and average age of 57.5 years (26 to 78 years). jaudice was the most frequent clinical finding (33/39) followed by RUQ abdominal pain (22/39) and pruritus (17/39), and palpable abdominal mass was noted on only 2 of 39. The carcinoma was
located at
the distal 1/3 of the EHBD in 59% of the patients and in 5.1%, 35.9% at the middle and proximal thirds, respectively. All cases were adenocarcinoma. Overall resectability was 69.2%. Twenty-seven of 39 underwent a resection (13
pancreaticoduodenectomy,
13 segmental resection, 1 pancreatoduodenectomy with trisegmentectomy of liver). Postoperative complications were wound infection (12.8%, n=5), andastomotic leakage (12.8% n=5), and UGI bleeding (2.6%, n=1). Operative motality was 15.4%(6/39).

3, 5-year survival rates in the curative group (92%, 73%, 61%, 40%) were superior to those in the noncurative group (50%, 0%).
The result of this study suggest that early diagnosis and surgical exploration with resection provides the only meaningful chance for cure and increased survival in these patiets.
